This card was given away for free from all Game TSUTAYA physical stores, and the TSUTAYA online store in Japan. It was given to anyone who purchased the Ocarina of Time 3D game beginning from its release date, while stocks lasted.

The card could be used by visiting the web address or scanning the QR code on the back of the card from any mobile device. People then had to enter the password (on the back of the card), and would then be able to download something (I'm not sure what). The card could only be used for a limited time (16th June - 31st July 2011), after which, the download was no longer available.
